I am not sure how to fill out part 3 of this form. Questions regarding the visa#, date issued, etc. I came to the U.S. 11 years ago on J-1 visa and changed it to F-1. I am still in school on F-1. I no longer have the J-1 visa because my passport expired and I was told I don't need the visa because it expired 10 years ago. They took the old passport, so there is no way for me to get to that visa anymore. I changed to F-1 in the U.S. so I only recieved letter of approval, not and actual visa #. I never traveled out of the country. I tried to get a copy of the J-1 from the embassy that issued it but they can't do that, so I was told. So, my question, if anyone can help me, how do I fill out that section on the form please?
