I too have a MSc from US university and planning to just send the transcripts of that and just list that as the university. Also regarding the English and other languages I am going to end up sending my 10th grade mark sheet. It lists all three languages with grades. I hope that should be enough as a certificate to prove my language efficiency. I took TOEFL before coming to USA but lost the certificate and dont have the scanned version of it. ETS website wont even keep the records as they delete it after 2 years.

Please tell me, if I have a different language certificate (IELTS), I will not be able to get a visa? I found in this article a comparison of two popular certificates https://preply.com/en/blog/ielts-vs-toefl/. Is only TOEFL required to obtain a visa?
