    i have health insurance through my employer, Health insurance is something that is typically renewed annually. For example, that's when "open enrollment" occurs

    renewal date is not mentioned anywhere, on the card or in documents.

    i am not sure what to put in the question "when does your health insurance terminate or date that it must be renewed?"

    any suggestions? thanks in advance

    Most people have health insurance through their employer. Open enrollment is typically the time period when they allow you to make changes which don't constitute "major life events". The problem is that if in the I-944 form you list the enrollment end date as the official termination date... that would mean you're saying that after the termination date, you won't have insurance anymore. But that's not true, it gets renewed automatically as long as you're still working there.

    I believe when they ask about the termination date, it's for the temporary health insurance that people sometimes buy (e.g. 1-year health insurance if you get laid off). There are some private health insurance options that must get renewed and expiration dates.

    For yours, I suppose you can leave it blank and in the "additional information", you can say it doesn't have an expiration date.
