H1B extension with no labor, I140 and I485 documents

Here is my situation...

• 2 1/2 years ago I filed my GC through company A
  
• I used pre-approved labor (I do not have a copy, company A is not providing me a copy)
  
• I140 and I485 filed together
  
• I140 approved (I do not have I140 approval notice, company A is not providing me a copy)
  
• I485 is pending for over 2 years (I do not have I485 receipt notice, company A is not providing me a copy)
  
• Biometric is done, and I have Biometric appointment notice
  
• After 6 months of filing I140 & I485, filed Advance Parole and EAD (with Biometric notice) and both approved/received
  
• An year ago changed job to company B on H1B
  
• AP and EAD both expired recently and filed for extention. I got AP receipt notice but EAD application is rejected twice (Reason mentioned is incorrect fee, but actual reason may be something else, I need to contact USCIS for more info)
  
• I am still on H1B status into 6th year (6 more months left)

Now I am wondering how would that effect my H1B extension after 6 years, knowing the fact that I do not have labor certificate, I140 approval notice and I485 receipt notice? Could someone please shed some light on this?

Also could some one please suggest me how to obtain copies of labor certificate, I140 approval notice and I485 receipt notice from company A? Otherwise can I request USCIS to provide me these documents? Any help is greatly appreciated.
