Birth Certificate frustration from Delhi

Hello All,
I am applying for AOS ( married to USC) , and was issued an RFIE for birth certitificate, despite providing a NABC. I have somehow managed to get a lawyer to represent me , and they are sharing a format like I have attached as the new format for my birth certificate. Is the indeed the format available from Delhi now?

My other question is : "I dont have proof of address for my parents at time of birth and these fields have been left empty, Can this be a source of more problems?

I’m from Chennai. The birth certificate I provided is almost similar to the image you posted except for the logo. I was able to pull it up online without any hassle. No issue so far with that BC (scheduled for interview for a year now).

you can check with your lawyer about the address part. I know name and spellings of you and your parents should match passport and other documents. If you need to change or add any information in the birth certificate you may have to contact local municipal board in Delhi. My friend had to do that for a typo in mom’s name.
