I am in the process of filing AOS application for my daughter under GC for international organization employee category. There is a requirement to provide

• An affidavit listing all entries and exits from the U.S. and the nature of the travel (whether for schooling, personal, approved customary leave, or business-related), if applicable

Can anyone point me to the sample affidavit template of form I can use to meet the above requirement?

I am not applying under the same category as you and hence I am not sure if my response would be of immediate help to you. Nonetheless, as part of my documents, I created a table where I documented all the dates I had entered and exited the US, the Visa category, the port of entry, and the purpose of travel. And I added well-labelled copies of my visas and the entry stamp for each entry.
