i485 & i765 appointment show my wife's name correctly, but after she received her employment authorization card today her last name is wrong. what can i do to fix this ? thank you

Yes, Enub4 is correct with that link, but only if the mistake was made by USCIS. If you made the mistake, then you have to refile and pay another fee. But since you said other documents from USCIS have had the correct name, then it's likely not your fault - but go back and check your original submissions anyway.Marriage AOS - 2018
