This is the list provided by my attorney. This is for EB1.
1. A copy of the long-form birth certificate of each applicant. The long-form birth certificate includes the names of both parents in addition to the date and place of birth of the applicant.
2. A copy of your marriage certificate, if applicable, and evidence of the termination of any prior marriage of each applicant, e.g., divorce decree, death certificate, if applicable.
3. Complete copies of all passports (current and previously issued) for each applicant including all U.S. visa and admission stamps as well as all blank pages. Please do not cut the copies of the passports down to the actual size of a passport.
4. Two (2) color copies of the biographic photo ID page of the current passport of each applicant.
5. Copy of the current I-94 card for each applicant (either hand written or electronic print out).
6. Copy of any and all Employment Authorization Documents (EADs) issued to each applicant.
7. A copy of your most recent Forms W-2 and Form 1040(no attachments or schedules needed, form only) for each applicant.
8. Any and all previously issued approval notices and immigration documentation available (e.g. Forms I-797, I-20, DS-2019, endorsed I-129S).
9. Six (6) passport-style photographs of each applicant which comply with USCIS photograph specifications. These photograph specifications may be accessed through the USCIS website at http://travel.state.gov/content/pass...-template.html. These photographs must have been taken within the past 30 days.
10. A completed medical examination for each applicant.
