Immigration Papers "Other names used"

Hi,
I am filling my immigration papers and on forms like I-765 etc there is a field:
OTHER NAMES USED...
I have changed my name several years ago (well just shortened it) and i provided the name change certificate from my origin country....
does that imply that i need to put my old name (that i dont use and only shows on my birth certificate) in that field? ....
and if i do will it appear on my new documents like green card, work authorization etc ?

I believe they need your old name for back ground checks.

I'm not sure, but I think you have to put all of the names you have ever used.
