Part 1 question 16 - Do you want the SSA to issue you a Social Security card?
Part 2 question 17 - I authorize disclosure of information from this application to the SSA as required for the purpose of assigning me an SSN and issuing me a Social Security Card.

Both are Yes/No questions, if I have already had an SSN, do I still answer "Yes" to both questions just to be safe?
