Where to submit I-485 after I-140 NIW approved?

I received the official approval letter (I-797) that my I-140-Adv deg or Extraordinary Ability NIW was approved. It said I must continue submit I-485 to Nebraska Service Center. However, after I read I-485 instruction, it seems to me that upon I-140 approval, I have to sent it to Phoenix Service Center. Does anybody have idea where or what service center I should submit my I-485 to? Should I follow I-797 instruction?

Another question is what box in Part 2 of I-485 I should pick?
