  • I-130 Other Names Used

    I'm the US petitioner for my wife's adjustment of status. I have a question about the I-130, Part 2 #5 Other Names Used. Suppose my full name is John Quincy Public, as recorded on my birth and marriage certificates and under Part 2 #4 Your Full Name. However, my US passport, foreign ID and foreign joint taxes use my middle initial, Q.

    Which is correct?

    Option 1:
    5.a. Family Name
    5.b. Given Name
    5.c. Middle Name

    Option 2:
    5.a. Family Name
    5.b. Given Name
    5.c. Middle Name Q.

    Option 3:
    5.a. Family Name Public
    5.b. Given Name John
    5.c. Middle Name Q.

    I would leave that blank because the middle initial is the same as the first letter of your middle name, so in my view, this is not an "other names used".

    If your name is John Quincy Public and you used John R. Public, then I will consider that as "other names used" but not using Q.

      There was a mess up with my name so they had to make a change and then the officer told me to put any name that i have ever been known as, even if its a spelling error because its a FBI check and they will know. So for you full name put first, middle, last then list all known variations of your name. I hope that helps.
