Hi

i recently Got my ead/ap card. I moved from company a (H1b) to company b(ead)
I am planning to travel to India and wants to know if any specific document is needed for reentry to US other than the EAD/Ap combo card.

anyone have experienced any issues or questions at port of entry?

Just your EAD/AP combo card should be good enough. You would be required to go through a secondary inspection at Port of entry. You would be asked to wait in a separate room while they run a background check on you. They might ask some basic questions about the nature of your visit, your work status in US etc. Otherwise nothing much. If you don't have any bad past, you should not have any problem. All the best.
