Can someone provide a list of documents to be provided with 485 AOS application for parents ? Do we need a medical exam to be submitted?

Did you read the I-485 instructions? What have you figured out so far? Basically:
$1225 check
I-130 receipt or approval notice (if filing I-485 separately from I-130)
passport bio page, page with visa used for entry, and page with entry stamp
I-94
birth certificate
2 passport-style photos
I-693 medical
I-864 Affidavit of Support

It wasn't clear if birth certificate was required because as per the country guidelines for India born, anyone born before 1970 or something, didn't have to produce a birth certificate. It wasn't clear if they need to produce another form of evidence like an affidavit.
Also, I didn't know if medical check was required for my mother in law who is more than 70 year old.
I thought there would be some post already in this forum where the requirements for applying green card for parents/in-laws would be discussed and someone like you might be able to share the link to the posts. I tried searching and couldn't find anything myself.

Hi, I applied for an AOS for my parent in March (please check latest requirements) and here's what I submitted:
G-1145 - E-notification of application
I-130 - You can do this online but I submitted a hard copy for concurrent filing.
Filing fee of $1,760 ($535 for I-130, $1,140 for I-485, $85 for biometric service)
Passport size photos of petitioner and applicant
Supporting documents including parent's birth certificate (or affidavit from a family member if you don't have it. I heard some people also provide board exam certificates or any official documents showing name and DOB), national ID card, passport bio page and visa, I-94, petitioner's birth certificate and passport showing proof of relationship.
I-485
For supporting documents, I provided duplicates of passport bio page, visa, etc just in case
I-864
Petitioner's tax returns, bank statements, proof of employment, pay stubs, and any other supporting documents
I-131 (optional)
I-765 (optional)
I-693 - Medical and vaccine report in a sealed envelope from USCIS-approved civil surgeon
