Vaccine exemption

Hi, asking for a relative. Slightly after a year from her interview she received a RFE - new medical form WITH covid vaccine. She isn’t vaccinated for religious reasons and of course will have her church provide with all necessary documentation that it is against her and their beliefs. Now my question is, i know the exemption is possible but can’t find anyone who has gone through this and would love is someone could comment on this and share their experience. Thank you

I think with non-compulsory vaccines her actions are still acceptable, but with mandatory vaccines that she doesn't follow, it needs to be checked.
