BirthCertificate - Help needed !

Hi Guys,
I got my 140 approval and going to apply for 485/AOS. I have birth certificate issues for both - myself and my wife.

Me - DOB is different from the one in all my records. Name is missing.

Wife - Name is missing.

Just providing affidavits with the original BC will suffice? or do I need to provide NON-Availability Certificate as well(Doesnt make sense though as BC is available but have problems)

If I get the name added to the BC and get a new BC, my wifes BC would contain her maiden name. How to work that out?

Any light on these issues would be highly appreciated.

You can produce non-availabitiy of birth certificate - i think there is a standard format - u need to get 4 - Rs20 stamp papers signed by parents and 2 other witnesses , which is accepted by uscis or contact the nearest Indian embassy whether they can give a birth certificate based on your passport - sslc ...etc.
