I was looking at the some of the forums for vaccination requirement. As most of us know, we would have been given vaccinations at some point in our lives back in our home country. I-693 requires information in list of required vaccinations along with Date they are administered.

    1. Is date information really needed?
    2. How can I get record of vaccination (or certificate) that I might have taken, back from the country I lived. Or is that something CS can determine here through the Blood test and than give me a list of vaccination needed.

Are you applying for AOS from K1 visa. If so you would have had a full medical before coming to the USA. Vaccinations can be checked by going to a local doctor and having a serology test done to see what your immunities are. If from K1 visa you should have been given a DS-3025 vaccination supplement and you take that to CS with I-693 and he fills out part 1., and 5 only transferring your vaccinations over to 693.

This is AOS for my parent who currenty has a visitor visa. So I will be filing I-130, I-485 concurrently when my parent is here in US.

Ok you can go to a local doctor and get that serology test done to see what immunities your parent has and then your medical cs will tell the rest as some wont be age appropriate.
When I came here to US I could not recall what I had either so I went to my local doc for the test and then if any is needed they will give them but it will be cheaper at a local doctor not the CS ok

First of all, what is home for you? As I know it does not matter if you had the immunizations back home. For USCIS: you have to do them from their designated Doctor. If your parents already in USA, you have to call immigration office to give you a list of surgeons or Doctor that they agree with; if your parents live back home, same thing, go the consulate or embassy for those information. I am saying this from my own experience. I have been living here in USA. As a female, I went through so many medical exams and immunizations, but this did not matter. Still with the records and every notes from my doctors , I had to go USCIS recommended Doctor, payed $385 (in 2005), redid all the vaccinations ( I had done through my life) and submit to USCIS for them to process my Green Card. Good luck, hope that will help you.

Home for me is Australia and I came here on K1 visa and did not have to have a full medical again as I had one before I came and it is valid for 12 months. I had my vaccination supplement paper with me and only had to have a flu shot, Depending on the age of this parent they will not need all vaccinations as some are not age appropriate. As it is a 485 and 130 yes they will need a full medical and they need to go to a CS recommended by USCIS. What I was saying was depending on age no they may not be able to find out all their immunisations from childhood but a local doctor can do that test before they go to CS. And you are talking about 2005 , it is now 2011 things are a little different. The medicals can cost a lot but they OP will need to check around first from the options they have in their area.

I see now! Thanks. Just check with the USCIS costumer services and see if they can answer your questions before going through regular doctors who can also mean money spending! Good luck to you all!
