Immi Help!! Original 797 Lost + Doc list for I485

  • Immi Help!! Original 797 Lost + Doc list for I485

    I am about to file for my I-485 (AOS) with my spouse. I am the primary applicant.
    My wife is presently on an H1, but I cannot find her original I-797 A, but
    I do have multiple copies of it. I am aware that I don't have to send in her
    original 797A or original I-94 (this is the lower portion) along with my
    application, but I do not have a copy of the back of the I-94 to be sent
    with my application.

    What should I do? Your response will be greatly appreciated.


    Also, these are the documents that I am planning to send in:

    For primary applicant:
    1. Check for $245.00
    2. Support Letter from Employer
    3. I-485 Appn. Form
    4. Biographic Information (4)
    5. 2 Adit Style Photographs
    6. Medical Examination forms (Sealed from physician)
    7. Copy of form I-94
    8. Copy of Approved I-140
    9. Copy of Birth certificate with 2 affidavits (mother, paternal aunt since name is not mentioned)
    10. Copy of pages from passport(s)
    11. Forms 9003
    12. Copies of last 3 paystubs
    13. Copy of degree certificate
    14. Copies of previous I797 (H1's)
    15. Copy of approved labor certification
    16. Copy of Marriage Certificate

    For my wife (derivative)
    1. Check for $245.00
    2. I-485 appn
    3. Affidavit of support (I-134) from myself
    4. Biographic Information (4)
    5. Medical Examination forms (sealed from physician)
    6. Copy of I-797
    7. Copy of I-94 (Only front in this case)
    8. copy of Birth certificate with affidavits (2)
    9. Copy of pages from passport
    10. Forms 9003 (2)
    11. Original bank statement (Joint a/c)
    12. Marriage Certificate


    Questions
    1. Should I send in copies of our last 2/3 tax returns
    2. Should I write a cover letter for my spouse or is the affidavit of support form (134) enough?
    3. My daughter was born here in the U.S. Should I send copies of her birth certificate?
