I-485 Birth certificate incorrect

Hi,

My birth certificate is in Hindi and it has the incorrect middle name.
e.g : Ajay Kumar Yadav . Actually my name is Ajay Sumit Yadav.
So, I have to do two things:
1) Translate the birth certificate in English.
2) Get some document for the name change.

Q1 : Do I have to get the translation done in my birth place only or I can get the translation done anywhere ?
Q2 : What document do I have to produce for the name change ? What is the process for that ?
