What are the supporting ducuments for filing I-765 for H4 dependant ?

Hi,

My husband is in H1. I have a previous EAD that has expired (I got it when I was in L2). Does anybody know what are the ducuments I need to submit for filing I-485 AND I-765 ? Please help.
