Sign and date each page of the application forms?

Hi to all, do i need to sign and date each page of the application forms? I'm about to send my packet.. Thanks to all who continuously are posting relevant info for other's information.

Make sure that you and your spouse sign and date all the forms. Do not leave any form unsigned. Sign and date all the forms and attached the needed pics and documents. Make sure you have the exact amount you need to send. The money order should have exact your fees required. No more no less. Even if it would be a multiple of money orders but the some should be what is required.
