Hi,
I got an RFE on 485 for secondary evidence on date of birth. Attorney suggested to get an affidavit from Friend, Aunt and Uncle and relative

Can any one please share me sample affidavit by Friend/Aunt/Uncle/relative

Affidavit

The affidavit has to identify the person first, it is better if it is signed by a U.S. Citizen or Permanent resident of the U.S. and then it has to specify the facts regarding your birth, it must be notarized.

Sample:

My name is , YYYY YYYY YYYY , I am a U.S. Citizen, xx years of age, residing at YYYY YYYY XX
in the city of YYYY XX state XX, zip code. The purpose of this affidavit is to state the following facts:

1) I am the YYYY YYYY (Uncle/Aunt) of YYYY YYYY XX (Your name) who was born on XX/XX/XX (date) at YYYY YYYY X (City/State/Country). To his parents YYYY YYYY YYYY (Name of father) and YYYY YYYY YYYY X (Name of mother).

2) I can verify this fact because Iived at YYYY ZZZ when he was born and I used to visit his home very often. I was also present at the hospital were he was born.

By signing below I attest to the fact that all of the above facts are true and correct to the best of my knowledge.
