Removal of conditions/ 10 year Green Card

On form I-751 there is a call for at least 2 people that have Known the couple since conditional residence to produce an affidavits of your relationship. We were wondering if anyone could give an example of said affidavits or has knowledge of the appropriate format? do these affidavits have to be notarized? Thank you

Affidavits Not Necessary

I can't answer your question but I've not seen anything to say that they need to be notarized. Samples can be found by googling.

What I do want to say is that affidavits are not necessary. We did not include affidavits and just received the wife's approval 2 days ago, 9 months to the day that we sent it in. The only time you should need affidavits, and I would be very suspicious if I was reviewing it, is if you can't adequately document your relationship.

What we included with the application:
- IRS transcripts showing joint returns
- Evidence of joint bank accounts
- Page from mortgage contract showing joint ownership
- Birth certificate of son (actually born prior to conditional green card)
