Green Card- Petition to Remove Conditions of Residence (I-751)

I would like to ask for helps and information regarding in Petitions to Remove Conditions of Residence (I-751). My renewal will be this coming August 2019.
Here are my questions:

1. How SOON should I will submit to USCIS? Could please elaborate to me, how and when did you submitted yours prior to your renewal dates/months (for those who has done with the renewal)
2. What is the complete address of the USCIS to submit my packets.
3. What are the requirements that I should include in my packet?
4. Does medical examination still required?

1) You can send in completed application 90 days before expiration of green card

2) Depends where you live, look it up in form instructions

3) Pretty much a continuation of your 485 packet. Look for TOC in this forum

4) No medical required
