Questions regarding filing Re-entry permit

Hello All,
I am a permanent resident filing Form I-131 (Re-entry permit) and have following questions:

1. In part 1 of the application, it asks for ""Class of Admission". What should I put here?

2. Do I need to include only the copy of my GC or some other document is also required? In the instructions document, it asks for an identification document also. Since Copy of my GC has the ID information so I believe that I don't have to include any other document like driver's license. Please confirm if my understanding is correct.

2.- including all information asked in the instructions (GC on both sides, and passport should suffice).
