Re-apply Green Card After Surrenderred

My mom had surrendered her green card years ago. Now she decided to re-apply. What kind of forms and documents are required for this process. Please advise the foam numbers.

By the way, my dad is a US citizen. Is it easier for her to apply Green Card through marriage?

Do we need an attorney? Where do we submit the forms--Home country or in USA? Does she need to wait at the location where submitted until interview?
