Filing out the separate I 751 form for a child petition

My daugher is a K2 and she achieved permanent resident status more than 90 days after the my (mother) K1 ( she fallow me in USA after 1 year) , so she will file her own I-751. She olso is 21 years old.

Please HEEEELP with filling out I 751 form !!!!!!!!!!!!!

Here is what I think should be:

Part 1 is my daughter name ( she is the one who need conditional removed) or mine her mother K1?
Part 2 . Checked 2 b and have a separate document where you will exapain why the dauther is filing a separate I 751 petition.
Part 3 is again my daugher????
Part 4 Is here my husband the US citizen or me, her mother K1 petitioner???????

Part 5 is the daughter who just file N/A because she doesn't have her own children.
Part 6 is just your daughter signature because she is not married ?

Please let me know if theis is correct and what shoud be filled out at those parts.

I apreciate any help because I can not afford to filed wrong this form. She need to travel outside US in December.
