Stay out side of US with GC for more than 6 months

I have a Green card and currently I am in India for almost 6 months. Can I stay out of US for more than 6 months but less than 1 year for medical reason.

Do I need re-entry permit or Should I need to go to US consulate? What document should I need to bring when I traveling back to US.
