Urgent - Help needed in filling I-90 Greencard replacement

I am about to renew my greencard which will be expiring soon. The section 3 is bit confusing to me. Appreciate any input.
Little background.
I got my H1B from US Consolate, Chennai, India. I landed in Chicago, IL and went to Detroit,MI and lived there till I got my green card. Applied green card from Detroit, MI. I belive the processing center was Nebrasca. I got my passport green card stamped at Detroit, MI
Question 1: I believe H1B is non-immigrant visa. Is it right.
Regd Section 3 in I-90
Question 2: City of Residence where you applied for an immigrant Visa or Adjustment of Status.
Is it Chennai, India or Detroit, MI
Question 3: Consulate where Immigrant Visa was issued or USCIS office where status was adjusted
Is it Chennai, India or Detroit, MI
Question 4: Date of Admission as an immigrant or Adjustment of Status
Is it the date I first entered US (@ Chicago, IL) or the date I got my green card approved or the date I got my green card stamped in my passport
Question 5: Destination in US at the time of Admision
I think it is Detroit, MI. Is it right
Question 6: Port of Entry where Admitted to US
I think it is Chicago, IL

Any input from you is greately apprciated
