What to fill in for Control Number on the Customs Declaration Form?

I am having Permanent Resident Card (Green Card) and travelling first time to India. My Question is what to fill in the Indian Customs Declaration from for the Control Number Field (MSC # or A#)?

Also will I be NRI? Can I check that option on the form?
