I need to go to Canada to get my passport stamped with my H1B visa renewal, so I am applying for a Canadian visitor visa.
When applying for the Canadian visitor visa, do I need to fill out the "Use of a Representative" form IMM 5476 if I am using an expediting service down in LA?
Also, it asks for the original I-94 card stapled in my passport, but I am going to the US Embassy in Canada so that I can get my new I-94 card along with my renewed H1B put into my passport. I have the I-797 showing my 1-yr extension with the I-94 at the bottom of that document, but should I send this original I-797, or just a photocopy of it to the Canadian Embassy in order to get a Canadian visa?
