How to use a letter regarding police certificate

I notice that the application package has a letter in the appendix B (obtain a police certificate). It starts with Dear Sir or Madam and bla bla. It seems to tell you present this letter to the police office to get police certificate. The latter part of the letter, you need to fill out the Authorization by visa applicant to release of police and court records. I do not understand what this part is for? Shall I fill out the form and give it to police office, FBI or just put it in in my application package to Canadian Consulate?
I appreciate someone who can address my question.

I think that part is only when the police or FBI questions why you need the finger print for. You can ignore that part and go ahead and apply for the PCC. It is enough if you explain why you need this PCC in the covering letter.
