EAD renewal queries-Pls help

For renewal of EAD and AP combo cards :

1. Should I-765 (For EAD) and I-131 (For AP) be filled and filed together in one envelope for one person? And the spouse's docs be in another envelope and couriered separately?

2. Should all docs for both husband and wife be put in one envelope and couriered?

Were you able to get this resolved? If so, I'd really appreciate if you could share the resolution.

Also,

what's the best way to include photographs with these forms? I feel that just clipping the photos may not be a good idea.
