renewal of employment authorization and TPS

Hi,
Do you choose a12 or c19 for question 16 of i-765? I am reapplying to renew my tps and EAD. I read the instructions few times and still couldn't decide which category I should choose. Also to question 11 Which USCIS office ? is it the Chicago office or vermont center as I live in NJ?
