Please Let me the answers for my doubts in applying H4 EAD

Hi,

My husband is on H1B extension. His i-140 is appproved now. I am on H4 VISA and planning to apply for H4 EAD. I have few doubts:

1. I see in website that EAD fee is $380. Does it include biometric fee of $80? if it does not include , then whom should i make biometric fee payment?

2. I live in north carolina. So is this the address i should mail my documents ? Because they have not mentioned particular address for the category C-26.so please confirm
