131 form : Questions on Photo Specs & Docs showing status

Hi,

I am applying Advanced payrole by myself and i am getting confused with two questions here

1. PHOTOS :
I know they are supposed to be two photos but are they supposed to be passport type photos showing the face in the middle or showing a three-quarter frontal view of the right side of the face, with your right ear visible

i got two different revewis from two different sites, which one is true?


2. DOCUMENT SHOWING MY PRESENT STATUS :

One of the requirements to send with the I-131 form is as per USCIS is

"A copy of any document issued to you by theUSCIS or former INS showing your presentstatus in the United States;"
with my AP form i am attached my H1 approval notice attached to it, but what should i send with my spouse's (H4 dependant) AP form, would it jsut be the visa or is it out marriage certificate and with it my H1 approval notice?

Please suggest, any help is appreciated.
