I-131 Question

My mother-in-law is visiting right now from Turkey. We have here here on an immigrant visa. She plans to return to Turkey in a few days and may not return for more then one year. We plan to file the I-131 on Monday and as I understand it, we are applying for a Reentry permit since she is a permanent resident, is that correct?

And it looks like we fill out blocks 1, 2, 3, 4, 5, and 8, right?

Any suggestions or advice is appreciated.
