Documetnation Required for EAD I - 765

Hi,

I am on L2, applying for EAD, can anybody let me know if any document other than the following is to be submitted along with I - 765.

1. Copy of I - 94 (front and back)
2. Copy of I - 94 (front and back - Spouse L1)
3. Copy to passport (first page, last page, visa page)
4. Check for $340
5. Two photographs
6.

You should submit the copy of receipt of I-485 or also I-140.It is also advisable to submit your Last L1/H1 approval copy.
