Need help to fill I-765

Hi
I am filling I-765 form.i got stuck at the question no 16
Already i applied I-485 and got receipt also.

16. Go to Part 2 of the Instructions, Eligibility Categories. In the space below, place
the letter and number of the category you selected from the instructions (For
example, (a)(8), (c)(17)(iii), etc.).

so how to fill this column?
and how much money do i need to write check for I-765
and what documents should i submit along with this application.?

What is your greencard based on? otherwise, i'd be taking a wild guess at what you need to put in there... provide more information.
