EAD and AP Renewal Questions

Hi Guys,

I have the following doubts when i am filling-up my EAD and AP renewal applications. Could anyone please give answers for the following questions.

I-765 (EAD): Question 11 :
which USCIS Office ? Date(s)
TSC ?
Which date i should write? (receipt date or notice date or granted date)

Question 14:
Manner of Last Entry? (visitor,student etc.,)
What should i write? H1B or Parole. (I used Parole during my last visit to india)

I-131 (AP) : Question 3:
Class of Admission?
What should i write? H1B or Parole. (I used Parole during my last visit to india)

I think you already know the answers. It is clear in the instructions and the form. Receipt date, Parole, Parole

I wasn't sure about the answers. Because some of my frienz attorney's wrote GrantedDate instead of ReceiptDate. Thats why i asked.
