checklist for renewal of EAD and AP

I'm looking for list of documents needed to submit for renewing EAD/AP. Can anybody post them...? I like to go for paper based filing...

It says I-131 application needs I-140 receipt notice...is that true? I do not have I-140 receipt. All I have is I-485, I-131(from last yr) and I-765(from last yr)...
