EAD renewal form filling doubts- Please help(urgent)

Hi,
I am on H1 and my wife is on EAD( EAD issued last year and I 485 is in pending status). I have following doubt for filling EAD for me and my wife.

for me - ( I am still on H1 and I485 is pending, not yet enabled EAD)
1) Quest# 10 -Alien Reg number - since Iam still on H1, is it EAD # or I-94 #?
2) Quest # 11 - Dates - which date do I need to enter? is it I765 notice received date or notice date or EAD start date?
3) Quest #15 - current immmigration status - is it H1 Visa?
4) Quest #16 -eligibility category - since my I485 is pending , is it (C)(O)(9)?

for my wife - ( she is working on EAD , I485 is pending and H4 is expiring in Ocotber , 2008)

Here are some responses based on my experience--

Q10: use A3 if you have receved EAD in past. I-94 not required for c-9 applicants.
Q11: use the receipt date given on notice received for filing of I-485
Q15: "ADJ of Status Applicant" for you and "SPOUSE/CHILD OF H-1 THRU H-3" for wife.
Q16: C-9
