Want information on name query(EAD)

Hello

My wife's name is surname:------ and Given Name: omar

but in VISA it got stamped as Surname: omar and Given name:FNU(for not in use)

I applied EAD as surname FNU (which is For Not in Use) and Given name : omar and got her EAD.

I got query on her EAD as below

"You must submit documentry evidence of legal name change from omar FNU to omar No Last name given"

Can anybody let me know what I would be submiting to answer this query?
