    Hello everybody,

    I have a problem, when I applied for a replacement EAD in March of this year, I had to get a new EAD due to USCIS making a mistake with my date of birth. I had to send my copy of birth certificate. Now for the I-765 replacement form I put in that I was born in county of Lomzynskie, just as it was stated in my certificate of birth. Now the problem I have is that that county was renamed to Podlaskie after two years after I was born and the only reason why I put in the form that I was born in Lomzynskie was to eliminate any possibilities of the immigration official seeing differing information. I was born in Poland by the way.

    MY EAD is expiring in 3 to 4 weeks, and I need to renew it. So what do I put in the form question 5 ( the county) Do I put in the new name of the county Podlaskie or should I put Lomzynskie again?



    PS;
    what documents are required to send in with my renewal?

    copy of I-485
    copy of my EAD
    two US type photographs
    check for $340
    copy of my passport

    anything else?

    Do these copies have to be notary certified?

    Also my other question is to the question 11 on the form asking me if I ever applied for the EAD and if so when?

    Does application for the replacement count? Or should I just put the date I originally applied for the EAD?

    I have one more question, where do I mail the applications if I live in New York City?

    Can I mail more than the application for renewal of all members of my family( 4 in total) in the same envelope? or 4 separate envelopes?

      Can be done in same envelop with clearly separating each package and check if possible in different envelops inside one big one.

      Many cities and towns change names. As long as you have proof of the name change from local authorities, it is acceptable and not an issue you need to be too worried about.
