I am not sure how to answer the question 11 about whether I ever applied for the work permit before. The thing is that in 2002 I worked at an American camp on J-1 visa as a student exchange summer program. I remember that we all have been applying for the social security number but I did not receive any SSN card or anything similar document so I do not have a number itself and any document stating it. Same applies to the EAD - I dont even remember applying for that - probably because the whole program was for 3 months only so there would be no way I could receive an EAD anyways.

I would very much appreciate your comments on what I should write in that form - considering my situation.

J1 is allowed to work without EAD. so go ahead and mark no to that question
