There is no special form for AC21 use. Your new lawyer can take care of it. It is informing USCIS about the change in employer, that your case is pending for more than 180 days, that your new job is same/similar to old job.

If you are unsure, you can hire a lawyer to do it for you. Employer could care less about it as it is your I485 petition, employer is definitely off the hook other than providing supporting documents.

I am not a lawyer and you need to consult with one to validate any info posted on the forum and discuss your case specifics. H1b Question? Read the FAQ first.

Google and look up forums on what needs to be done. Informing USCIS about AC21 is a simple thing and many people do it themselves. You just have to do some hoework and gather information from the internet forums. There is already plenty of info available on the forum. I think I might have even posted a sample. Do your homework or hire a lawyer. May cost 150-500 $$. It is half an hour worth of work. No big deal.

A letter that identifies the relevant sections of the law that allows you to use AC21, a new offer letter from your "permanent" employer, a clarification that your job remains the same/similar to which the labor was filed, your A#, 485 recpt, etc should be sent to USCIS so that they do not deny the I485 in case the ex-employer revokes the I140.

Search and read the AC21 memo here or from google. That will explain.
