EAD I-765 Form question regarding lost EAD from OPT
Hi, I lost my EAD card from my OPT that was granted in 2004. I'm now (Feb 2009) applying for EAD on pending I-485.
How should I answer question 11 of the I-765 Form since I don't know the USCIS office, the date, and don't have it to attach it ?
