Query abt "Place of Last Entry into US" for EAD form filling application

I have one question regarding "Place of Last Entry to US". I am on Dependent L2 visa.I came from India and my port of Entry in US on 28th Feb 2010 was "Newark" . But DHS office had bymistaken wriiten " H4" on my I-94 form. When I reached Seattle, I saw my I-94 form and I corrected it by again going to Seattle Airport, as Seattle was also the Port of Entry in US. I am filling I-765 for EAD. Now my question was what should I write in "Place of Last Entry to US" column. Is it Seattle (Bcoz I corrected my Visa status their) OR is it Newark. In my I-94 FORM, it is written " Department Of Homeland Security. US Customs and Border Protection. ADMITTED SEA" and date is my entry of US i.e. "FEB 28,2010".
Can someone plz give me a quick reply on this issue?

Place of 'last' entry

Hi. Reading your query i understand that you have two seals in ur I94 form one of newark and the other of seattle. i believe that ur seattle entry is the correct and the 'last' (and latest) entry to US. So if this had been my case i would go for seattle. Please do confirm the same with some1 else too. please dont rely on this mere suggestion of mine alone coz am also an amateur who applied for EAD last week. One question i have for u. the port of entry also gives a DHS seal on the passport with date and other details. even there u ve two seals of newark nd seattle?

Seattle will be treated as deferred inspection. That is the right answer.

EAD - Place Of Entry

I am applying for my Wife's EAD.

We entered United States through Washington and got the I-94 stamped there. Later after arriving at Des Moines (final destination in US) after a week we got the I-94 corrected by a USCIS offcer for an extension.

However the corrected I-94 mentions "ADMITTED DSM" while earlier it was "ADMITTED WASHINGTON" What should I mention as place of entry in I765. Des Moines Or Washington?

Further the eligibility criteria for L2 Visa holders. there are three fields at Section 16 like () () () in the I765.
Should I enter (a)(18)() i.e.. leave the third one blank or it should be like (a) (1) (8)

Please help me to complete the form I765.
