Some insurance companies cover the USCIS immigration medical exams. Fill up the forms on your insurance website and send it to the insurance companies along with the doctors receipt. Its good if the insurance pays for it.

Medical Examination - 485 Requirements
Just go to your local family doctor and get the required tests done as per 485 requirements.
Take the results of these tests and your arm with the TB shot to the USCIS designated doctor.
You do not have to mention to the insurance company what these tests are for, particularly. You regular medical insurance allows certain diagnostic tests without a substantial medical illness requirement.
This will reduce your final costs drastically.
I've known many to use this option and they save a bunch of money, unless of course the money grows on trees in your lawn, then no worries!
