I am a physician who has applied for Greencard via EB 2 route. My I-140 is approved and by February 2009 I will have 6 months completed since I filed I-485. I intend doing a fellowship in July 2009. Does anyone know if one can switch from a job to a fellowship in a subspeciality they are currently practicing under AC 21 rules ?

Yes, it is a problem. Talk to your attorney.
If you started out as a Hospitalist for the GC labor, the next job for riskfree AC21 use should be as a Hospitalist only, not as a PCP. The same applies to your situation.
